About the role
Diverge Health is seeking a Practice Consultant to build and maintain relationships with primary care practices, supporting onboarding, workflow improvement, and use of technology and care solutions to enhance value-based care outcomes for Medicaid patients.
What you’ll do
Build and maintain relationships with primary care practices
Support onboarding and workflow improvement
Help practices leverage technology and care solutions
Interpret reports and dashboards to track performance and close quality gaps
Identify opportunities for improved outcomes
Manage multiple projects simultaneously
What you’ll need
Bachelor's degree or equivalent in business, healthcare administration, or related field
2–4 years of experience in healthcare operational or customer-facing role
Strong understanding of value-based care models and HEDIS measures
Familiarity with risk adjustment methodologies (HCCs)
Proficiency in Microsoft Office, especially PowerPoint and Excel
Ability to manage multiple projects and effectively communicate with providers and payers
